Jul 8, 2009 - You can't download the forms from the site anymore, you'll have to click the "Upon Request" link to have them mail you one. Or you can use their electronic office to do it online. When you copyright them online it's $10 cheaper than mailing it, and the process is faster. Hope this helps!
