Women with larger breasts produce milk at slower rates, since they can store more. But both small and large-breasted women produce the same amount on average. Breasts can grow after pregnancy, but the change isn't always dramatic, and only stays as long as the mom nurses, which in Western countries is about 1-2 years. That child seems older than that.

Lactation begins after the birth of a child, so pregnant women will not secrete milk from their breasts. The liquid the breasts do leak is actually colostrum, NOT milk. However, with hormone therapy and breast pumps, it is possible to make breasts secrete milk, with or without pregnancy (it's called induced lactation.)
